12:00pm-1:30pm: Awards
1:30pm-2:15pm: Yearbook Singing & Pictures

RSVP + TICKETS
(Please Read Carefully)
There are 2 required steps to attend the banquet.
STEP 1: COMPLETE THE RSVP FORM
• Enter your athlete’s name to RSVP for them.
• If you are attending with your athlete, you must add yourself as an additional guest
• Add ALL guests attending (parents, grandparents, siblings, friends, etc.)
Important:
• Do NOT include children age 2 and under (they are free and do not need an RSVP)
• All guests age 3 and older MUST be included in the RSVP form
STEP 2: PURCHASE TICKETS
• You must purchase a ticket for each guest listed on your RSVP (age 3+)
• DO NOT purchase a ticket for your athlete (athletes are FREE)

Every person attending (except children 2 and under) must be:
